This morning, Aran Salut, Servicis Asistencianaus Integrats, and the TIC Salut Social Foundation signed a collaboration agreement to establish the lines of action to implement the Person-centered Care (PCC) model promoted by the General Council of Aran. This PCC is funded by the Next Generation programme. The meeting between Jordi Vilana, Managing Director of Aran Salut, and Joan Guanyabens, Director of the TIC Salut Social Foundation, represents the starting point for the implementation of integrated care in the Aran Valley.
The PCC model will be applied through the Programme for the Development and Implementation of the Person-centered Care Model. Organisations such as the Catalan Government Departments of Social Rights and Health, the Catalan Hospital Union Foundation and the Polytechnic University of Catalonia are involved.
The aims of this project are to consolidate the management of social and health care in a unique and integrated way, and to respond to the logic of empowering people with more autonomy and quality of life.
The actions are aimed at the design and development of more proactive care through predictive models for the early identification of situations of social vulnerability and/or clinical complexity that allow for personalised care pathways to be proposed.
The care system will be modernised through an ecosystem of technological and digital solutions combining robotics, home automation and telemedicine. Home care, as an area of action, will allow technological deployment with the aim of extending people’s autonomous and independent lives.
