The Departments of Health and Social Rights and Inclusion have launched a new Action Protocol for data communication between the public healthcare system and the social services network. The goal is to strengthen collaboration between professionals in both fields and provide more coordinated, comprehensive, and person-centered home care.
As Conxita Barbeta, Head of Integrated Social and Health Care at the Department of Social Rights and Inclusion, highlighted during the closing session of the Technology to Promote Social and Health Care conference held on July 22, the approval of this protocol is key to sharing knowledge and data while following security procedures and ensuring technological robustness that foster coordination between departments.
The protocol enables healthcare and social teams to share relevant information about each individual in order to conduct a multidimensional assessment and establish a single care plan that brings together all necessary resources. This prevents citizens from having to manage administrative procedures themselves and ensures that they are at the center of the care process.
This improvement will especially benefit people with complex health and social needs, as it facilitates the coordination of care and services both during home care and after hospital admission.
Technology is a crucial element in this process. Digital platforms make it possible to exchange clinical and social information between professionals and local entities in a secure and efficient way.
Thanks to this shared environment, it is possible to improve:
Ultimately, the inclusion of both social and health data in the same technological environment contributes to better health and well-being outcomes, more efficient use of public resources, and progress towards a more sustainable and integrated health and social care system.
