Within the framework of developing the Person-Centered Care Model of Aran, the project aims to implement a unified response to the health and social needs of the population, promoting their active participation in decision-making through:
Integrated and multidisciplinary case management (integrated assessments and action plans across social and healthcare areas, as well as individualized approaches designed to enhance the person’s overall experience).
Technological solutions (innovations in home automation, robotics, and telemedicine).
Data management in care processes (using artificial intelligence systems and knowledge tools to analyze and predict citizens’ needs).
Support solutions (to facilitate and promote empowerment and independent living for people with greater clinical and social needs).
These efforts reflect an adaptation to change and the adoption of innovative practices to ensure a more efficient and equitable healthcare system, fostering proactive, integrated care that incorporates new technologies.
Of Aran’s particularities, we highlight its full competencies in health and social matters, a unique funding model provided by the Conselh Generau d’Aran, a single legal framework, and one collective labor agreement for all professionals. This unified approach reflects the political will to implement a model change that encompasses all the necessary aspects to provide high-quality care.
This project is an integral part of the guidelines established by the Catalonia Health Plan 2021–2025 and the Catalonia Strategic Plan for Social Services 2020–2024, where Person-Centered Care is identified as a core element. Moreover, collaboration and partnerships with entities such as Fundació Unió Catalana d’Hospitals, Fundació TIC Salut i Social, Suara Cooperativa, and the Polytechnic University of Catalonia reinforce the commitment to research, innovation, and continuous improvement to ensure quality care for all citizens of the Val d’Aran.
The government of the Conselh Generau d’Aran and the Servici Aranés de Benèster e Salut have long been implementing improvements aimed at the definitive establishment of an integrated care model — the Person-Centered Care Model of Aran — in which the individual plays an active role in the planning, development, and evaluation of the care process, with the goal of achieving the highest possible levels of health, quality of life, and well-being, while respecting each person’s dignity, rights, needs, and preferences.
