ADMIT - Home Integrated Care

ADMIT is a project that aims to change the current way older adults are cared for, moving towards a model that integrates social and healthcare services. The goal is for people to live at home for as long as possible and with a better quality of life, especially those who are frail or have complex needs.

The project proposes several actions to offer more person-centred care, with professionals from the health and social sectors working together and in a coordinated way. All of this is supported by a technological platform that enables professionals, the person receiving care, and their environment to stay connected and collaborate more effectively. The project is funded with FANF resources from CatSalut.

ADMIT aims to ensure that people can spend 5% more time living in their own homes, improving their quality of life and helping to reduce or slow down frailty through:

  • Improving people’s well-being at home based on prevention and personalised approaches to frailty.

  • Enhancing the experience of professionals and caregivers through training, coordination, and reduced administrative burden.

  • Making the system more sustainable by using technology and supporting informal (non-professional) caregivers.
