Integrated Social Health and Services (SISS) is a pioneering project to improve care for users of the Home Care Service (SAD) in Manresa.
This is the case of the shared computer system (Alexia) that encompasses both the user’s social and health information, promotes collaborative work and improves communication between primary care professionals (Health) and those in the SAD (Social), and also between the user and these professionals.
To provide the best possible care to people in situations of dependency and/or complex needs who are users of the Home Care Service (SAD) in Manresa and also of the city’s primary care centers.
