The session was about integrated social and health care within the framework of the Master\u2019s Degree in Integrated Management of Services in Primary, Social and Hospital Care (GESAPH) and was held by the Institute of Continuing Education &#8211; IL3 at the University of Barcelona.<\/p>\n<p>During the event, Mr Vilana took part in a roundtable discussion on leadership in local areas with Ms Teia F\u00e0brega, Director of the Garrotxa Social Action Consortium; Ms Ester Gavald\u00e0, Director of Primary Care for Terres de l&#8217;Ebre (Catalan Health Institute); and Ms Elisabeth Ortega, Director of the Ripoll\u00e8s Social Welfare Consortium.<\/p>\n<p>During the debate, the Person-centred Care Model (PCC) promoted by <em>Integrated Care Services of Aran<\/em> to integrate social and health services was presented. This is a <strong>unique model that presents a vision of a pioneering country <\/strong>that is promoted through the <em>Integrated Social and Health Home Care Programme in Aran. <\/em><strong>The TIC Salut Social Foundation is taking part in this project <\/strong>with other organisations such as the Uni\u00f3 Foundation and the Polytechnic University of Catalonia.<\/p>\n<p>Mr Vilana highlighted the importance of strategic alliances with the ecosystem. He emphasised that Aran, with its high mountain geography and seasonal demographic demand, is a key region to gain knowledge and valuable experience in the shared path towards integrating social and health care throughout Catalonia.<\/p>\n<p>Aran\u2019s PCC Model provides numerous benefits to citizens, professionals and organisations by offering equity, quality of life and greater efficiency in service provision.
