The Departments of Health<\/strong> and Social Rights and Inclusion<\/strong> have launched a new Action Protocol for data communication<\/strong> between the public healthcare system and the social services network. The goal is to strengthen collaboration between professionals in both fields and provide more coordinated, comprehensive, and person-centered home care<\/strong>.<\/p>\n\n\n\n

Integrated and personalized care<\/h3>\n\n\n\n

As\u00a0Conxita Barbeta<\/strong>, Head of Integrated Social and Health Care at the Department of Social Rights and Inclusion, highlighted during the closing session of the\u00a0Technology to Promote Social and Health Care<\/a><\/em>\u00a0conference held on July 22, the approval of this protocol is key to sharing knowledge and data while following security procedures and ensuring technological robustness that foster coordination between departments.<\/p>\n\n\n\n

The protocol enables healthcare and social teams to share relevant information about each individual in order to conduct a multidimensional assessment<\/strong> and establish a single care plan<\/strong> that brings together all necessary resources. This prevents citizens from having to manage administrative procedures themselves and ensures that they are at the center of the care process<\/strong>.<\/p>\n\n\n\n

This improvement will especially benefit people with complex health and social needs<\/strong>, as it facilitates the coordination of care and services both during home care and after hospital admission.<\/p>\n\n\n\n
