Last May 2017, the Fundaci\u00f3 TIC Salut Social and Novo Nordisk Foundation signed a collaboration agreement to promote the Diacare project, seeking benefit for people with type 2 diabetes, as well as their personal and healthcare environment.<\/p>\n
The aim of the project is to promote self-management and self-care by people with this, most prevalent, type of diabetes. In fact, of the more than 5.3 million people with diabetes in our country, between 90-95% have type 2. This platform of services, via mobile technologies and based on the profile of the person with type 2 diabetes, will therefore provide patients with support and supervision tools, allowing decisions to be taken for the optimal monitoring and control of their diabetes. Help and monitoring tools will nevertheless be available for health professionals and caregivers.<\/p>\n
The Diacare project, developed at the CAP Comte Borrell, is currently in the design phase of solutions to improve self-management of type 2 diabetes mellitus. For the solutions to be appropriate, they will be carried out through co-design, based on the needs obtained in the Patient Journey carried out through various workshops with patients and professionals to obtain a true overview. These workshops were conducted to understand the patient’s route through the primary care system, taking into account all the interactions that the patient has, from different points of view.<\/p>\n
On the last day of the Trends Map meeting, we had the chance to know first-hand the evolution of this project that is part of the Foundation\u2019s line of digital transformation, thanks to the account given by the patient, Mrs. Maria Escudero and Doctor Eduard Tarrago, both of whom are involved in the project.<\/p>\n
About the Diacare platform<\/strong><\/p>\n Among the main services that are being evaluated in a platform such as Diacare are: a patient profile tool to define their needs and specific goals in accordance with the main health indicators in diabetes (level of glucose in blood, medication and lifestyle); a decision support service adapted to their profile and to which health professionals and caregivers will also have access; an interactive diary for self-check of glucose in blood, medication, nutritional habits and physical exercise; and a follow-up and intervention service by health professionals to monitor the state of the person with diabetes and to trigger the necessary protocols in the event of emergency.<\/p>\n","protected":false},"author":21,"featured_media":3366,"menu_order":0,"template":"","meta":{"_acf_changed":false,"inline_featured_image":false},"etiqueta":[],"tipus":[],"topic":[],"class_list":["post-3369","noticia","type-noticia","status-publish","has-post-thumbnail","hentry"],"acf":{"imatge":3367,"autor":"Adri\u00e0 G.Font","textos_destacats":[{"texte":"On the last day of the Trends Map meeting, we had the chance to know first-hand the evolution of this project that is part of the Foundation\u2019s line of digital transformation, thanks to the account given by the patient, Mrs. Maria Escudero."}]},"yoast_head":"\n
