TIQUE PCP Final Event
On 19 May the TIQUE PCP Final Event will take place, an initiative focused on improving care for people with advanced heart failure through an integrated, person-centred, value-based approach.
Throughout the day, the main results and lessons learned from the project will be shared, as well as the experience of implementing the digital tools TIQUE V and Plat4TIQUE in real clinical settings in Spain, Sweden and Italy. The session will also address the challenges identified, future opportunities and the importance of organisational coordination to ensure a real and sustainable impact of digital innovation on healthcare.
