The i2X project, Intelligent Implementations of the European Electronic Health Record Exchange Format, aims to advance towards an interoperable electronic health record at European level, enabling citizens’ health information to be shared safely, accurately and comprehensibly across different countries and healthcare systems. To make this possible, the project implements the European Electronic Health Record Exchange Format (EEHRxF) in real clinical settings and works across five priority data categories defined by the European Health Data Space Regulation.
Running for 48 months, from April 2025 to March 2029, i2X brings together 38 partner organisations across 12 EU Member States. Ten countries are actively deploying solutions in 14 operational healthcare settings, based on use cases defined by clinicians, IT teams and patients. Implementations are based from day one on standards such as HL7 FHIR, IHE profiles and EU Implementation Guides, with the aim of achieving effective interoperability in day to day clinical practice while reducing the burden on healthcare professionals.
The mission of i2X is to enable every EU citizen to share their health data safely and accurately with any clinician, anywhere in Europe.
Specifically, the project aims to implement and validate the European Electronic Health Record Exchange Format in real clinical settings; build systems that are interoperable by design and grounded in European and international standards; validate the five priority data categories defined by the European Health Data Space; provide clinicians with patient information that is complete, structured and ready to use, regardless of the country of origin; enable citizens to access and share their health records in any EU country; and provide Member States with tested tooling, validated use cases and a community of practice to support national rollout.
The TIC Salut i Social Foundation actively participates in i2X by supporting the adoption of the European Electronic Health Record Exchange Format in Catalonia. Its role focuses on advancing the implementation of the Patient Summary at Clínica Girona and Clínica Bofill and piloting it with real patients, while also incorporating discharge reports. The Foundation also leads the work on medication and ePrescription, enabling private healthcare providers to generate interoperable medication plans.
The project is co funded by the European Union under the European Union’s Digital Europe Programme under grant agreement No 101195323. It aligns with the Digital Single Market strategy and supports the ambitions of the Digital Decade 2030.
